Please bring a full body, posed picture of the dancer and a head shot along with this application. (3x5 of 4x6 sizing is acceptable).
Fee of $25 will be charged when you sign up through your parent portal. If you did not pay this fee through your online registration, it will be collected at the studio prior to the audition.
